Nicely done!
Obviously the face and expression are the most important things in a portrait (and you've nailed them here) but the extra little details of the fabric creases, the crisp bit of text and the shading of the arm really push this one to another level.

Yes, the light looks very natural as it falls across the skin. I can never get it quite as subtle as that, perhaps because of that pain you speak of. :)

I was gonna leave that comment until I posted another pic in the thread but they've all been going wrong at the moment so we'll just forget about that idea. :lol:
